Abstract:
Objective
To change the model of hand over in stroke ward in order to utilize nurses'work time.
Methods
Modifying the traditional model of hand over, morning meeting attendance for all nurses becomes optional instead of mandatory.This new method requires all charge nurses enter the ward 15 min earlier to see their patients'situation,make nursing plans and nurse their patients.Nurses who are about to off duty need to describe and explain patients'situation under their duty area to nurses who are about to start their work by turn.In stead of asking questions after morning meetings,the head nurse will give a summary to today's work and assign new works for tomorrow before off duty.Head nurse as an instructor,an auditor and a supervisor,needs to follow the track of all her team members,and provide help if needed.
Results
The new hand over model shortened the hand over time by at least 45 min.This new model not only gave the charge nurses more time to caring her patients,but also relived the pressure of treatment during noon time.Meanwhile,by the testing results(provide in a table below),the proficiency of nurses increased by roughly 7%.And patients'degree of satisfaction increased by 3.2%.
Conclusions
The new hand over model shorten the time to hand over the next shifts, increase all nurses'work efficiency,fulfill patients and their families'need and increase patients'degree of satisfaction.
